What is home telemonitoring?
Home telemonitoring is a Texas Medicaid health service that requires scheduled remote monitoring of
patients’ vitals related to a client’s health.
The transmission of the data from the client’s home to a
licensed home health agency must comply with standards set by HIPAA. Data parameters are established
as ordered by a physician’s plan of care. Data must be reviewed by a home health registered nurse (RN),
NP, CNS, or PA, who is responsible for reporting data to the prescribing physician in the event of a
measurement outside the established parameters.


Telemonitoring Purpose:
The home telemonitoring software is customized with a patient’s distinctive conditions and monitors
patients vitals in real-time.
The daily vital signs recording updates are used to help the patient’s
physician adjust the treatment plan as needed.


How it works:
Every day or as ordered, home health nurses will monitor patients’ vital signs and symptoms remotely
from a patient’s home to the home health agency to identify abnormal readings and address negative
symptoms or health concerns before they turn into medical emergencies. When necessary, the nurse will
communicate the findings to the ordering physician.

Telemonitoring software allows the home health team to:
 Monitor patients’ vitals in real-time
 Respond to high-risk readings that could lead to medical emergencies or admissions into the
hospital
 Communicate with patients via telephone or text messages
 Export logs of patients’ vitals and send by fax or email to the physician monthly or as requested.
 Allow ordering physicians to log into the telemonitoring software to access patients’ vitals.

Who qualifies?
Home telemonitoring is a Texas Medicaid benefit for clients who have been diagnosed with either
diabetes or hypertension or both and meets qualifying risk factors.
Telemonitoring services will not be approved for clients of any age who have diabetes or hypertension
unless they have two or more of the following risk factors:

 Two or more hospitalizations in the previous 12-month period
 Frequent or recurrent emergency department visits
 A documented history of poor adherence to medication regimens
 Documented history of falls in the previous 6-month period
 Limited or absent informal support systems
 Living alone or being home alone for extended periods of time
 A documented history of care access challenges
Home telemonitoring services is also a benefit for clients who are 20 years of age and younger, with one
or more of the following conditions:

 End-stage solid organ disease
 Organ transplant recipient
 Requiring mechanical ventilation
The physician who orders home telemonitoring services has a responsibility to ensure the following:
 The client has a choice of home telemonitoring providers.
 The client has the right to discontinue home telemonitoring services at any time.

How do I get my patients started?

The Agency’s staff will:
1. Obtain informed consent to treatment from the patient.
2. Get prior authorization from client’s insurance.
3. RN will gather, review and document information for the physician’s plan of care.
4. Install the home telemonitoring device in the patient’s home.
5. Educate the patient on the use of the telemonitoring device.
6. Monitor the patients’ vitals as ordered by the physician.
7. Report any abnormal readings to the ordering physician.
